Arthroscopy: A Perfect Saviour From Joint Problems

Joint pain is a common work-related condition that can severely affect one’s ability to perform daily activities. Joint pain can affect anyone irrespective of profession, age, and sex.

In a national survey, about one-third of adults were reported to have been suffering from joint pain. It is found that as you get older, painful joints become more common. Joint pain can range from mildly irritating to severely debilitating. People usually manage joint pain with medication, physical therapy or alternative treatments.

If you have a chronic pain your doctor can suggest arthroscopy. They usually recommend this for treating inflammation in joints, injured joint, or damaged joint over time. Arthroscopy is commonly done on the knee, shoulder, elbow, ankle, hip, or wrist.

arthroscopy kerala

Arthroscopy is a procedure in which a surgeon inserts a narrow tube through a small incision. This tube is attached to a fiber-optic video camera which allows the surgeon to see inside your joint without making a large incision. They can even repair the damages in the joints during arthroscopy, with pencil-thin surgical instruments inserted through additional small incisions.

Conditions treated with arthroscopy include:

  • Loose bone fragments
  • Damaged or torn cartilage
  • Inflamed joint linings
  • Torn ligaments
  • Scarring within joints
This is a very safe procedure and uncommon complications.  Problems that may rarely happen include:
  • Tissue or nerve damage as a result of the placement and movement of the instruments within the joint. This can damage the joint's structures.
  • Infection. This includes any type of invasive surgery.
  • Blood clots. This is a very rare condition that appears when procedures that last longer than an hour can increase the risk of blood clots developing in your legs or lungs.

When does an ACL injury requires surgery?

One of the primary ligaments in the knee joint is the anterior cruciate ligament (or ACL). For normal movement and mechanics of the knee joint the ACL is important. People may notice problems in the way their knee functions when the ACL is torn.

Common symptoms include:

    • Knee swelling
    • Joint Pain
    • Buckling sensations
    • Instability
    • Difficulty to perform sports activities

To replace a torn anterior cruciate ligament, ACL reconstruction or surgery is performed. ACL injuries most commonly occur during sports such as soccer, basketball, downhill skiing, football, and gymnastics that involve sudden changes and stops in direction.

During the surgery the torn ligament is removed and replaced with a piece of tendon from a deceased donor or from another part of your knee. It’s performed through small incisions around your knee joint and is an outpatient procedure.

Why it’s done?

ACL injuries happen when you put stress on the knee during sports and fitness activities.

For individuals who are relatively inactive, or engage in moderate recreational activities and exercise a course of physical therapy may successfully treat an ACL injury.

ACL surgery is generally recommended if:

    • You're an athlete and your training involves pivoting, cutting or jumping
    • More than one meniscus or ligament in your knee is injured
    • During everyday activities your knee is buckling
    • You are young and physically very active

ACL injuries can either be partial or complete. It is difficult to decide on surgery when there is a partial ACL tear.

Based on how much the ligament is damaged ACL sprains are graded by doctors. If there is only minor stretching to the ligament it is Grade 1. A partial ACL tear where the ligament is damaged and loose is Grade 2. If there is a complete tear of the ACL it is Grade 3 sprain, which is the most common. Surgery is almost inevitable in a Grade 3 injury as the knee joint becomes unstable.

A decision is taken by most surgeons based on how much instability the injury has caused. Surgery is recommended if the knee is unstable. Non-surgical treatments may be considered if the knee is stable.

The decision is usually based on physical examination findings and analyzing a combination of symptoms. If you have episodes of feeling the knee giving way or knee instability, that can be a determining factor. The tests done in the physical exam will be considered by the doctor. These tests often include pivot-shift test and the Lachman test. Knee reconstruction is a reasonable option if these are consistent with laxity of the ACL.

Commonly Affected Areas Due To Osteoarthritis

Pain in the joints makes it difficult to enjoy a normal life. Every time you walk, run or climb up and down the stairs, a sharp pain always accompanies it. With age, the tissues and muscles in the joints weaken and lose its flexibility. Osteoarthritis is marked by degeneration of joint cartilage and the underlying bones. It results in pain and stiffness in the joints, especially in the hip, knee, and the joints of the thumb. Osteoarthritis is one of the most common forms of arthritis that affect people belonging to the middle age group. By maintaining a healthy weight and staying active, it is possible to slow down the progression of osteoarthritis and improve joint function.

Common symptoms of Osteoarthritis

    • Any activity like walking or bending is always accompanied by pain.
    • When applied pressure, joints become tender.
    • Joints are always stiff after any activity.
    • The joints are not as flexible as they used to be.
    • The movement of the joints is always accompanied by a grating sound.

Where does Osteoarthritis affect the most?

Osteoarthritis can affect any joint but it usually affects those joints that had experienced repetitive stress or injury. Following are the joints most commonly affected by osteoarthritis:
  • Knee- it is the common lower-limb joint that is affected by osteoarthritis. Osteoarthritis in the knee can cause severe pain while walking, running, and climbing the stairs. If not treated, this can lead to the disability of the knees.
  • Hip- osteoarthritis in the hip leads to pain in the areas surrounding the hip. A severe hip pain hinders movement and causes a lot of inconveniences.
  • Hand- a person with osteoarthritis in the hand usually has the disease running in their family. It could result in pain and swelling in the joints of the fingers and severe pain at the base of the thumb.

Does Knee Pain Interfere With Your Daily Life??

Surgical treatment for knee pain is known as knee transplant. It’s of two types.

Partial knee transplant : In this method, the surgeon replaces only the damaged portions of the knee with metal and plastic parts.

Total knee replacement : In this procedure, the complete knee is replaced with an artificial joint. It requires a major surgery and hospitalization.

http://orthopaedic-surgery-india.com/knee-replacement/

Who needs a knee replacement surgery?

If the knee is really stiff and painful, difficult to perform even the basic activities, only then will the knee replacement surgery be considered. This surgery is mainly reserved for people over the age of 50, and those who have severe osteoarthritis.

What happens during knee replacement surgery?

Once administered with the anaesthesia, an 8 to 12-inch incision is made in the front of the knee. The damaged part of the joint is then removed from the surface of the bones, and the surfaces are then shaped in such a way to hold a metal or plastic artificial joint.

The artificial joint is then attached to the thigh bone, shin and knee cap with the help of cement or any other special material. When fit together, the newly installed artificial parts of the joint, will be relying on the surrounding muscles and ligaments for support and function.

How is the recovery period after knee replacement surgery?

The minimum time of stay in the hospital after knee replacement is mostly three to five days. Mostly the vast majority of patients who have undergone the knee joint replacement surgery have dramatic improvement. The improvement is most notable one month or more after surgery.

The pain caused by the damaged joint is completely reduced when the new gliding surface is constructed during surgery. After the knee replacement, people are standing and also moving the joint the day after surgery.

Orthopaedic Surgery India is a super speciality clinic, which offers various orthopaedic treatments in Chennai. The treatments include Joint replacements, Arthritis, Sports Medicine.

How To Heal Ankle Sprain?

Sprain is defined as the tearing of ligaments that connects one bone with another and help stabilize the joint. In such cases, the ankle joint may become unstable and take a long time to recover if not treated properly.

A sprain in the ankle can cause swelling, pain and limited motion. Many people tend to simply treat a sprained ankle at home with rest, ice and pain relievers. Whereas some severe sprains cannot be taken care at home and may need medical evaluation.

Causes

The ankle joint is a hinge joint formed between bones of the lower leg and the bone of the foot which allows it to bend upwards and downwards with a small amount of rotation. The ligaments provide stability for the movement. Any injury to this ligament results in swelling and pain on outside of the ankle.

The injury can get worse in case of previous or existing ankle injury especially if poorly rehabilitated, lack of strength and stability in the ankle or extreme flexibility, poor balance and also the increasing age of the individual.

Diagnosis

One should consult an efficient orthopaedic in case of pain that is severe and uncontrolled, patient is unable to walk at least 4 steps, ankle shows no signs of improvement within five to seven days, foot or ankle looks misshapen, patient experiences a loss of feeling in the foot or toe.

Sprains are categorized and treated depending on the degree of ligament tear.  They are graded as 1, 2 and 3 as per their condition as mild, moderate, and severe respectively. The injury is dealt with great care with the aim to reduce the bleeding and damage within the joint.

The standard protocol followed for treating a sprained ankle has been the R.I.C.E. method - Rest, Ice, Compression, and Elevation. This protocol typically prevents further damage and allows a gradual healing of a sprained joint. It is found that sprains tend to heal most effectively when the injured joint is exercised as soon as possible. Elevate your injured ankle and rest it on a blanket or pillow while on the couch - this can help promote fluid drainage. While your ankle is elevated, try to spell out the alphabet with your foot. If the injury is severe, you will not be able to do it at all.

Dr. Thadi Mohan, Consultant Orthopaedic Surgeon, provides treatment for sports related injury, trauma management and other orthopedic conditions such as osteoarthritis, ACL injuries, tennis elbow, cuff tear, low back pain and joint replacement with his skilled team and best in class amenities.
